Job Description

We are actively seeking a dedicated Family Practice Physician for our client in Minnesota, for a 248-day assignment commencing Oct 5, 2026. This vital role focuses on delivering comprehensive primary medicine care, including preventative services, acute care, and chronic disease management, within a consistent daily schedule from 08:00 a.m. to 4:30 p.m.


Responsibilities and Duties

  • Provide Primary Medicine Care, promoting a team-based, patient-centered approach to health care.
  • Deliver Preventative Care, including annual physicals, screenings, and vaccinations.
  • Offer Acute Care, treating common illnesses and minor injuries such as colds, flu, rashes, and sprains.
  • Manage Chronic Diseases, helping patients manage long-term health conditions like hypertension, high cholesterol, and diabetes.
  • Provide Health and Wellness counseling and patient education on healthy lifestyle choices, nutrition, and weight management.
  • Conduct Diagnosis, Evaluation, Treatment, Care Management, and Consultation across all settings for various conditions including chronic and acute conditions, hypertension, diabetes, hyperlipidemia, joint pain, and mental health concerns.
  • Be present on time for any scheduled clinics.
  • Provide consultation and instruction to referring Physicians regarding appropriate indications for procedures.
  • Determine appropriate courses of treatment and communicate with referring clinicians.
  • Initiate appropriate social work referrals for identified homeless veterans and patients without primary care providers.
  • Follow all established medication policies and procedures.
  • Provide discharge education and follow-up instructions coordinated with the next care setting.
  • Communicate all test results in accordance with VHA Directive 1088(1).
