Job Description
Our client in Minnesota seeks a dedicated Family Practice Physician for a 248-day assignment. This role involves providing comprehensive primary medicine care, focusing on preventative, acute, and chronic disease management. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m., ensuring a consistent work-life balance.
Responsibilities and Duties
- Provide preventative care, including annual physicals, screenings, and vaccinations.
- Treat common illnesses and minor injuries for acute care needs.
- Manage chronic diseases such as hypertension, high cholesterol, and diabetes through regular check-ins, medication management, and ordering tests.
- Offer health and wellness counseling, patient education on healthy lifestyle choices, nutrition, and weight management.
- Perform diagnosis, evaluation, treatment, care management, and consultation for a wide range of conditions including chronic and acute issues, mental health concerns, and various ambulatory conditions.
- Provide consultation and instruction to referring Physicians regarding appropriate indications for procedures.
- Determine the appropriate course of treatment and communicate with referring clinicians.
- Initiate appropriate social work referrals for identified homeless veterans and for patients who do not have primary care providers.
- Adhere to all established medication policies and procedures.
- Provide comprehensive discharge education and follow up instructions coordinated with the next care setting.
- Communicate all test results in accordance with VHA Directive 1088(1).
