Job Description

We are seeking a dedicated Internal Medicine Nurse Practitioner for a 90-day assignment in Massachusetts, commencing on May 4, 2026. This role involves providing comprehensive primary care to PACE participants, managing chronic and acute medical conditions, and active participation in interdisciplinary team meetings. The schedule is typically weekdays, from 8:00 AM to 5:00 PM, focusing on patient care coordination and thorough documentation in the electronic medical record system.


Responsibilities and Duties

  • Provide comprehensive primary care to PACE participants
  • Manage chronic and acute medical conditions
  • Participate in interdisciplinary team meetings
  • Coordinate care with nursing, social work, and therapy staff
  • Document care in the electronic medical record
  • May include clinic visits, home visits, nursing home coverage, or phone/in basket coverage
