Job Description

We are seeking a dedicated Family Practice Physician for an urgent 248-day assignment in Georgia, starting Jul 1, 2026. This role involves providing comprehensive primary care, managing chronic and acute conditions, coordinating patient care within a PACT model, and ensuring thorough documentation. The schedule for this vital position is typically 8am to 4:30pm, focusing primarily on direct patient care for veteran beneficiaries.


Responsibilities and Duties

  • Provide comprehensive primary care, including preventive services, chronic disease management, and acute care evaluations.
  • Conduct mental health screenings and make appropriate referrals.
  • Serve as a team leader in the Patient Aligned Care Team (PACT) model, coordinating with specialists and managing referrals.
  • Promote health and disease prevention through patient education and use of clinical reminders.
  • Integrate geriatric and palliative care principles, addressing aging-related issues and collaborating with relevant teams.
  • Offer women's health services, including gender-specific care and trauma-informed approaches.
  • Utilize telehealth and virtual care methods, such as video visits, telephone appointments, and secure messaging.
  • Maintain accurate and timely documentation in the electronic health record, complying with VA Policies and performance measures.
  • Review all inpatient admissions recommended by Advanced Practice Providers and ensure proper hand-offs.
  • Provide expert consultation and instruction to referring physicians regarding appropriate procedures and treatment plans.
  • Initiate social work referrals for identified homeless veterans and patients without primary care providers.
  • Adhere strictly to all established medication policies and procedures.

  • Case Types: Chronic and acute conditions in veteran beneficiaries, including hypertension, diabetes, and other service-related conditions. Specialty referrals are expected when necessary.
