Additional Information

  • Must possess an active state dental license without restrictions.
  • Required Board Certifications: Academy of General Dentistry (AGD).
  • Other certifications: Basic Life Support (BLS).
  • Candidates should provide a copy of a valid license, NPI number, CV/Resume, CPR/BLS certification, real ID driver’s license, and NPDB report.
