We are seeking an experienced Hospice/Palliative Medicine Nurse Practitioner for a locum tenens assignment in Illinois. This opportunity begins on Nov 3, 2025, runs for 128 days, and features day shifts Monday through Friday, 8:00 am to 4:30 pm. The position involves providing home healthcare to hospice patients across private residences, nursing homes, and long-term care facilities.

Responsibilities and Duties

  • Evaluate patient status and document clinical findings
  • Monitor and record vital signs

  • Complete patient recertifications and write prescriptions as needed
  • Adapt to a changing weekly patient list across various care settings


Additional Information

  • Active Illinois nurse practitioner license required
  • Experience in palliative and hospice care is a plus
  • At least 1 year of post-graduate experience in critical care, medical-surgical, emergency room, or urgent care settings preferred
  • Familiarity with the Aprima EMR system is beneficial
