We are seeking a Hospice/Palliative Medicine Nurse Practitioner for a 128-day locum tenens assignment in Illinois, starting Nov 3, 2025, with day shifts scheduled Monday through Friday. This role involves providing palliative home healthcare, managing critical cases, and evaluating patients for potential hospice care in private homes, nursing homes, and long-term care facilities.

Responsibilities and Duties

  • Evaluate patient status, including vital signs and medication management
  • Conduct home visits, typically seeing patients twice monthly
  • Assess and determine if patients require transition to hospice care
  • Write prescriptions and manage medication refills
  • Complete patient recertifications
  • Collaborate with home nurses or facility staff as needed
  • Document care using the Aprima EMR system


Additional Information

  • 1 year of post-graduate experience required
  • Experience in critical care, med-surg, emergency room, or urgent care preferred
  • Palliative and hospice experience a plus
  • Must be licensed as a Nurse Practitioner in Illinois
